To obtain a medical waiver, please make an appointment with your health care provider and have them complete a Medical Contradiction Form.
Virtual Non-Medical Exemption (Waiver) Process
The Mid-Michigan District Health Department (MMDHD) has implemented a virtual non-medical exemption (waiver) process for your convenience.

You must complete the entire education session. Once finished, you will be automatically directed to a secure JotForm link where you will provide permission for MMDHD to complete your non-medical waiver in the Michigan Care Improvement Registry (MCIR):
If you choose not to accept the electronic signature option, you must contact the MMDHD branch office nearest to you to schedule an in-person appointment to complete the non-medical waiver process.
Please allow up to 3 business days for staff to process your request in MCIR before contacting your school or daycare to confirm completion.
